Payment & form must be returned by SUNDAY, NOVEMBER 16, 2014

• Digital ads are MANDATORy: .wmv, .mov, or any compatible movie/video file

• Your business card will be featured in the program book! Please be sure to submit your card

following the same guidelines listed in the program book ad form.
